Please note that permission slips are required FOR ALL EVENTS, but if your teen is attending more than one event, then only one medical release and one photo release will be required. Please download and complete all the necessary forms, scan them, and email them to Jenn Kollasch (jkollasch@assumptioncatholicchurch.org).
